Who Needs to Enroll:

Kentucky requires children between 6 and 16 years old to attend school. Children can enter primary school at age 5 if they turn 5 on or before August 1.

A child may sign up for the preschool program if your child turns 4 before Oct. 1 and is income-eligible. Also eligible for preschool are children age 3 or 4 who have a special need. A child with a disability is one who demonstrates a delay in one or more areas of development.


To enroll a child at Second Street School, families should go to the school office, located on the first floor from 7:00 am to 3:30 pm. Once your child is registered, they will be referred to the school counselor for enrollment.


Requirements for Enrollment:

  • Guardian’s Drivers License (Required) – must have in-district address
  • Proof of Residency (Only if no drivers license) – rental contract and/or utility bill that shows guardian’s name along with in-district address
  • Student’s Official Birth Certificate (Copy)
  • Student’s Social Security Card (Copy)
  • Proof of guardianship - if child does not live with natural parents. Educational guardianship is not accepted.
  • Completed Enrollment From
  • Kentucky Immunization Certificate – up to date with all required immunizations
  • Kentucky School Physical – must be given within one year of beginning public school. Another health care examination is required within one year prior to entry into the sixth grade.
  • Eye Examination – by an optometrist or ophthalmologist is required for children entering public school for the first time.
  • Dental Examination – by a dentist (K - 1st)

Kindergarten Registration:

  • Original Birth Certificate
  • Copy of Child's Social Security Number
  • Free Lunch Form (When appropriate)
  • Proof of Residency - street address which may include utility bills to the legal guardian, rental/lease/purchase contracts, etc.
  • Guardian’s Drivers License (Required) – must have in-district address
  • Proof of guardianship - if child does not live with natural parents. Educational guardianship is not accepted.
  • Court documents pertaining to your child (if applicable)
  • In compliance with the Kentucky State Board of Education and Public Health regulations, parent/guardian must (on or before the first day of school) have provided the school with the following:
    • Kentucky Immunization Certificate showing all age appropriate vaccines including: Diptheria-Pertussis-Tetanus (DPT) and Polio Vaccine (IPV or OPV) last dose given after Age 4, two doses of Measles-Mumps-Rubella vaccine (MMR) second MMR due at age 4, three doses of Hepatitis B vaccine (HBV), Chickenpox vaccine (Varicella) one dose or a note stating that child has had the disease.
    • School Medical Examination Form completed by family physician within 1 year before first time school enrollment. Preschool exam will be acceptable for Kindergarten.
    • First time enrollees to Kentucky public schools also require an eye examination performed by an Optometrist or Ophthalmologist.
    • Dental Examination – by a dentist

Tuition and ADA Applications:

We urge all new and prospective families that live outside of our district to complete tuition and ADA forms. The forms can be attained from the link below . Please return the completed forms to the Second Street School office.
